How to get your 2022 license? ​

I. ONLINE LICENSE

• First go to  https://licence.ffsa.org/  then create an account.

​

• Bring a credit card, your license number (renewal) and a valid email. Before taking your licence, you can have your medical certificate signed by your general practitioner or a sports doctor.

• Then just follow the instructions below:

​​​ II. PAPER LICENSE (ALWAYS AVAILABLE)

​

   1. COMPLETE AN FFSA 2022 FORM

• Download the FFSA 2022 Form  and complete pages 4 and 6  (ASA code = 0305).  

​

• In all cases, complete the right part of this form "Optional Individual Guarantee" = if you do not subscribe to any formula, tick the box "I do not subscribe" and sign this part.

  2. MEDICAL VISIT  

​

• You can contact either a general practitioner regularly registered on a board of the Order of Physicians, or a sports doctor. The doctor will have to stamp your form in the two boxes provided for this purpose. 
   

  3. CONSTITUTION OF YOUR FILE

​

• Return your completed license application form, accompanied by a check payable to "Asa Mirecourt" (only 1 check for the amount of the FFSA license  + ASA contribution 35€) to the following address:
